Patient Information

Scheduling Your Appointment

When you call to schedule your appointment, please have your insurance card accessible so that we can obtain your member ID, policyholder name and date of birth and claims mailing address. Our staff will also ask if you have a latex allergy, a heart murmur or any artificial joint replacements. 

 

Your Appointment 

When you arrive for your appointment, we will get you checked in and give you paperwork to begin the registration process. Please make sure to bring your driver's license and insurance card as well as any x-rays and notes that your general dentist may have given you. Additionally, if you are taking any medication on a regular basis, please bring your list of medications (with dosages listed) with you. Any applicable copays will be due at the time of treatment; copays will not be billed. 

 

If you would like to expedite your registration process, you may download some of your patient registration forms to fill out in advance and and bring with you to your appointment. There will be one additional portion to fill out in the office:

 

 

 

 

 

 

 

Cancellations 

Please notify the office of any cancellations within 48 hours of your scheduled appointment.
